Safety in the use of medications: difficulties of detection of the medication errors and some proposed of prevention

Abstract

The problem of the medication errors has been waking up interest in the scientific community motivated by the high incidence. In the occurrence of errors it is necessary a systematic verification seeking to explain the causes and starting from this, to establish preventive strategies. This study has as objectives to explain the difficulty in to evidence the incidences of the medication errors and to present some preventive strategies. It is a bibliographical rising, being used the bases: Medline, International Pharmaceutical Abstracts (IPA), International Nursing Index (INI), Scielo and Lilacs. The discoveries were organized, analyzed and synthesized, evidencing like this, the main circumstances that they involve the identification of the errors and the preventive proposals considered conclusive. We believed that for us to reach a reduction in the medication errors is necessary that professionals of the health, academy, researchers, institutions of health and the State if they become aware of the importance of the problem and acquire a culture based in the systemic approach and in methods that reduce the medication error.
